CFS Awareness Day Special | Expert Care at Rehab Station Physiotherapy Clinic in Pune
Living with constant fatigue that doesn’t improve even after rest can feel frustrating and confusing. On this Chronic Fatigue Syndrome Awareness Day, it’s important to understand a condition that often goes undiagnosed— Chronic Fatigue Syndrome (CFS/ME)
What is Chronic Fatigue Syndrome (CFS)?

Chronic Fatigue Syndrome is a long-term, complex medical condition characterized by severe fatigue that:
- Persists for more than 6 months
- Is not relieved by rest
- Worsens with physical or mental activity
Unlike normal tiredness, CFS affects multiple systems of the body, including the brain, immune system, and muscles, making even simple daily tasks exhausting.
In cities like Pune, where busy lifestyles are common, many people mistake CFS for burnout or stress—delaying proper diagnosis and treatment.
Signs and Symptoms of CFS
CFS symptoms vary from person to person but typically include:
Persistent Fatigue
A deep, ongoing exhaustion that limits work, exercise, and daily routines.
Post-Exertional Malaise (PEM)
A hallmark symptom where even minor physical or mental effort leads to a worsening of symptoms, often lasting 24–72 hours or more.
- Sleep Problems
- Feeling unrefreshed after sleep
- Disturbed sleep cycles
- Cognitive Difficulties (“Brain Fog”)
- Poor concentration
- Memory lapses
- Slower thinking
- Pain and Physical Discomfort
- Muscle aches
- Joint pain without swelling
- Frequent headaches
- Other Symptoms
- Dizziness on standing (orthostatic intolerance)
- Sore throat or tender lymph nodes
Diagnostic Criteria of Chronic Fatigue Syndrome
There is no single test to diagnose CFS. Diagnosis is based on clinical evaluation and exclusion of other medical conditions.

Standard Diagnostic Criteria (IOM/CDC Guidelines)
A person is diagnosed with CFS if they have:
- Severe Chronic Fatigue
– Lasting 6 months or longer
– Not due to exertion
– Not relieved by rest - Post-Exertional Malaise (PEM)
- Unrefreshing Sleep
- Plus at least one of the following:
Cognitive impairment
Orthostatic intolerance
Ignoring symptoms of CFS can lead to:
Reduced productivity and work capacity
Mental health challenges like anxiety and frustration
Increased physical deconditioning
Early diagnosis allows for better symptom management and improved long-term outcomes
Physiotherapy Treatment for Chronic Fatigue Syndrome
Physiotherapy plays a critical role in managing CFS safely and effectively. The goal is not to “push through fatigue,” but to restore function without triggering symptom flare-ups.
- Energy Management (Pacing Techniques)
Pacing, initially described by health psychologist Ellen Goudsmit in 1989, advises patients to: ‘do as much as you can within your limits’
Patients are taught how to:
Balance activity and rest
Avoid overexertion
Prevent crashes (PEM episodes)
Interventions were of varying durations, from 3-weeks consisting of 1 session/week pacing self-management, to 3 × 60-90-min sessions/week of ‘activity pacing self-management’, a 6-week rehabilitation programme using the activity pacing framework (6 × 3.5-hours sessions), and 1-year of activity pacing therapy
This is one of the most important strategies in CFS management.
- Graded Activity & Exercise Therapy (Carefully Supervised)
Starts with very low intensity movements
Gradual progression based on tolerance
Focus on consistency, not intensity
⚠️ Important: Exercise is customized—overexertion can worsen symptoms if not guided properly
- Breathing & Relaxation Techniques
Helps reduce fatigue and improve oxygen efficiency
Supports nervous system regulation - Postural Training & Ergonomics
Correcting posture reduces unnecessary energy expenditure
Essential for IT professionals and desk workers in Pune - Pain Management
Gentle mobilization
Stretching exercises
Soft tissue techniques - Functional Rehabilitation
Training for daily activities (walking, climbing stairs, work tasks)
Improves independence and confidence
When Should You Visit a Physiotherapist?
You should seek help if you experience:

- Fatigue lasting more than 3–6 months
- Difficulty performing routine activities
- Symptoms worsening after mild activity
- Unexplained body pain and brain fog
Early physiotherapy intervention can prevent deterioration and improve quality of life.
